Emergency laparoscopic surgery saves a patient with a ruptured ectopic pregnancy.

Ectopic pregnancy rupture is an emergency condition in obstetrics that poses a serious threat to a woman’s life if she does not reach the hospital in time and if diagnosis and management are delayed.
Recently, Hai Phong International Obstetrics and Pediatrics Hospital performed laparoscopic surgery to save a patient with a ruptured ectopic pregnancy.
Previously, patient P.T.H, 38 years old from An Lao, Hai Phong, arrived at Hai Phong International General Hospital in a state of lower abdominal pain, particularly in the uterus and both adnexa, lasting since the afternoon of the previous day. Upon initial examination, the doctors at Hai Phong International General Hospital recognized the patient’s critical condition and quickly invited BSCKI. Nguyen Van Huynh – Deputy Head of the Obstetrics Department to conduct a consultation right in the Emergency Area. After the consultation, BSCKI. Nguyen Van Huynh concluded that the patient had an ectopic pregnancy with signs of rupture and needed to undergo emergency laparoscopic surgery immediately to ensure the patient’s safety.
The patient was immediately resuscitated while preparations for the surgery were made. The surgical team included BSCKI. Nguyen Van Huynh – Deputy Head of the Obstetrics Department, BSCKII. Pham Thi Loan – Obstetrics Department, and the anesthesia team led by BSCKI. Nguyen Do Hung – Head of the Anesthesia and Resuscitation Department.
Upon entering the abdominal cavity, the surgical team discovered a significant amount of blood and clotted blood inside, estimated at about 1200ml. At the isthmus of the right fallopian tube, there was a 4x4cm ruptured gestational mass that was bleeding into the abdominal cavity. Additionally, a 3x3cm dermoid cyst was found on the patient’s right ovary. The surgical team quickly performed cauterization and excision of the gestational mass at the right fallopian tube while controlling the bleeding and dissecting the dermoid cyst, preserving the healthy part of the right ovary for the patient. Typically, in cases of severe and complex pathology with both a ruptured ectopic mass and an ovarian tumor, doctors would prioritize managing the gestational mass first rather than risking simultaneous management of both the mass and the tumor. However, with high expertise and extensive experience, along with skill and talent, the surgical team at Hai Phong International Obstetrics and Pediatrics Hospital successfully addressed both challenges simultaneously. Just 35 minutes later, the surgery was completed successfully and safely. Patient H received a total of 02 units of blood during the surgery, overcame the critical situation, and was transferred for monitoring in the Obstetrics Department at Hai Phong International Obstetrics and Pediatrics Hospital.
