Rare occurrence: Successful surgery to deliver a pair of twins sharing the amniotic sac and placenta with a cord entangled in multiple knots.

Recently, Hai Phong International Obstetrics and Pediatrics Hospital successfully welcomed a rare case of twin pregnancy with a shared placenta and shared amniotic sac, where the umbilical cords were intertwined into knots, resulting in a safe delivery, with “mother round and baby square.” The mother, P.T.T, 21 years old from Thanh Ha, Hai Duong, chose Hai Phong International Obstetrics and Pediatrics Hospital as the place for examination and delivery during her first pregnancy. After discovering that mother T was carrying twins, particularly a case of shared placenta and shared amniotic sac, which is extremely rare, occurring in about 1 in 40,000 pregnancies in general and 1 in 10,000 twin pregnancies specifically. Associate Professor, Doctor of Medicine, Level II Tran Thi Viet Phuong – Director of the Obstetrics Department stated that a shared placenta in twin pregnancies is already dangerous, and having a shared amniotic sac is even more perilous, increasing the risk of congenital disabilities and the risk of sudden fetal death due to the possibility of twin-to-twin transfusion syndrome. Additionally, if there is a shared amniotic sac and the two umbilical cords are close together, when the two fetuses move back and forth, they can easily intertwine, creating knots. The more knots that form and tighten, the more they will gradually cut off the mother-fetus circulation, leading to fetal heart loss at any gestational age. The risk of death in the group with a knotted umbilical cord is 4-5 times higher, and this pregnancy complication cannot be predicted in advance.
