Successfully saved a 6-year-old patient who suffered severe blood loss due to a rupture of a small intestine vascular malformation.

Recently, Hai Phong International Obstetrics and Pediatrics Hospital successfully saved a child with severe blood loss due to a ruptured vascular malformation in the small intestine – a rare gastrointestinal condition that, if not treated promptly, can lead to death.
On June 2nd, 6-year-old patient N.T.K from An Duong district, Hai Phong was admitted to the hospital in a state of massive bright red blood in the stool, pale skin, no vomiting blood, rapid pulse, and low blood pressure. Blood tests showed that the hematocrit (HCT) was 18%, and the hemoglobin (Hgb) level was 58g/l (normal values are 110-133g/l).

Through the initial examination, patient K was diagnosed with severe acute blood loss due to lower gastrointestinal bleeding. Immediately, the patient was transferred to the Pediatric Intensive Care Unit for consultation and was instructed by the doctors to continue blood transfusions, laboratory tests, and imaging studies to identify the causes of the bleeding. In the Pediatric Intensive Care Unit, the patient was diagnosed with acute blood loss due to small intestine bleeding. An emergency consultation was conducted during the shift under the direction of Dr. Pham Van Duong, Deputy Director of Pediatric Specialty, who ordered both resuscitation and surgery to ensure the patient's life. The surgery was performed by Dr. Hoang Van Quynh from the Pediatric Surgery Department along with Dr. Do Manh Ha from the Pediatric Intensive Care Unit, and Dr. Nguyen Thi Ha from the Anesthesia and Resuscitation Department, who operated on the abdomen to locate the bleeding point. Due to significant bleeding, the blood volume in the segment of the small intestine close to 1 meter and the entire colon made it very difficult to observe and identify the bleeding point and any abnormalities. After a period of searching, the doctors identified a suspected bleeding site within the small intestine.
