Emergency surgery for simultaneous two conditions: ruptured appendicitis and Meckel’s diverticulum for a pediatric patient using laparoscopic method at Hai Phong International Obstetrics and Pediatrics Hospital
Recently, the International Pediatric Center, Hai Phong International Obstetrics and Pediatrics Hospital performed a double surgery, simultaneously addressing two conditions: a ruptured appendix and a Meckel’s diverticulum.
The patient, V.B.A, 4 years old, from An Duong, Hai Phong, was admitted with abdominal pain and fever for 2 days prior to admission. After examination and tests, the doctor diagnosed the patient with late appendicitis and indicated an emergency laparoscopic surgery.
During the surgery, the appendix was found to be located behind the cecum (the first part of the large intestine) and was ruptured with pus. The surgical team immediately performed laparoscopic dissection to expose and remove the appendix, cleaning the abdominal cavity. Upon checking other organs in the patient’s abdomen, a Meckel’s diverticulum was discovered. With the family’s consent in the operating room, the patient underwent simultaneous laparoscopic resection of the Meckel’s diverticulum. The surgery concluded safely, and the patient was treated postoperatively in the pediatric surgery department, stabilizing and has since been discharged.

The appendix located behind the cecum is a very rare abnormal position that is difficult to diagnose using conventional imaging methods such as ultrasound and clinical examination. The common cause of appendicitis in young children is due to the incomplete absorption of the yolk sac connecting the fetus to the yolk sacs during pregnancy, which poses a risk of forming a Meckel’s diverticulum. If this condition is not diagnosed and treated promptly, it can be very dangerous; the appendix may rupture and cause peritonitis, retroperitoneal inflammation, or require open surgery.
